   Sec. 402.305(4)(a), Florida Statutes: Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. A ratio of [2] child care personnel for [12] children is required. Ratios/Discipline [SR] Comments: The following ratios were observed: Infants 1:4 1's 2:11 2's 1:12 ratio was brought into compliance when additional staff member arrived. New ratio = 2:12 3's 1:15 VPK 1:12 A ratio of [ 1 ] child care personnel for [ 12 ] children was observed. Upon entering the classroom, the children were found running around and climbing the bookshelves while the teacher was setting up cots. Another child ran into a bookshelf bumping his head. The licensing monitor asked the teacher who was working with her. The teacher stated she just arrived and did not know who was working with her nor did she know who was covering the classroom prior to her arrival. The licensing monitor brought this concern to the Office Manager. An additional staff member arrived to assist the children in the classroom, bringing this violation into compliance.    Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [the licensing monitor observed a 2-year-old wandering into the front office]. Ratios/Discipline [SR] Comments: At the time of inspection, the licensing monitor observed a 2-year-old wandering into the front office. When the licensing monitor and office manager walked the child back to her classroom, the teacher stated she was unaware the child left the room. The monitor advised the provider to keep toddlers' classroom doors closed, and place safety covers on classroom doorknobs. Child care facilities with a licensed capacity of more than 50 children shall have one additional drinking fountain located either inside or outside the facility for each additional 50 children licensed capacity or any fraction thereof. Manual or electrically powered commercial water coolers may be used to meet the indoor drinking fountain requirements. Electrically powered commercial water coolers may be used to meet the outdoor drinking fountain requirements.    Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. License/Certificates [SR] Comments: During today's inspection the monitor found inaccurate attendance recordings in the 2's classroom.
