Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. 3.1(A), Handbook: All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Areas of the facility were found to be in need of cleaning. 3.10.2(G), Handbook: Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall be placed on the diaper changing table. [Children's bottles, sippy cups, and food items] was observed being stored on the diaper changing table. Toilet/Bath Facility [SR] Comments: At the time of inspection, licensing monitor observed items unrelated to diaper changing stored on and underneath the changing table including children's food items. The cabinet door on the changing table also needs repair. This is the second non-compliance of this standard and a Notice of Violation is issued. This standard was previous numbered as 27-08. 2.4.4(B), Handbook: Children must not be left in high chairs or other types of feeding chairs other than feeding times. A child was observed in a high chair outside of feeding times. Food Service/Nutrition [SR] Comments: At the time of inspection, licensing monitor observed [2] children in the one-year old room who were in highchairs beyond feeding time. Personnel stated that one child is "busy" and so child was left in highchair while personnel fed another child. Personnel stated that one child does not walk and so child was left in highchair while personnel fed another child. Both children were removed at the time of inspection. Licensing monitor observed two highchairs in the cafeteria area, one highchair in the three-year-old room, and one highchair outside of the kitchen door. 6.5(I), Handbook: Prior to administering medication to children, child care personnel responsible for administering medication must be educated on proper administration procedures. Written documentation must be maintained in the personnel file that child care personnel administering medication have been educated on proper administration procedures. Staff did not complete or have documentation of necessary training. Childrens Records [SR] Comments: At the time of inspection, licensing monitor observed an incomplete No.5 medication authorization form for prescription eyedrops and Tylenol belonging to a 1yr old child. Personnel stated that Tylenol is on "as needed" basis but did not remain at facility. Personnel stated they did administer eyedrops as indicated. Personnel administering medication did not have documentation of medication administration training. At previous inspection, provider stated medication was not administered by facility. 7.1, Handbook: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. 3.8.2 (A), Handbook: Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. The facility has not been inspected and approved by the local fire department as required. License/Certificates [SR] Comments: At the time of inspection, a Notice of Violation has been issued by the local fire department due to alarm panel failure and provider has been placed on mandatory fire watch.
