3.8.4(A) and (C), Handbook: During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care. License/Certificates [SR] Comments: Licensing monitor arrived at facility to complete reinspection and conduct an observed fire drill. Per Director, drills are normally conducted utilizing a hand bell. Director states "I don't really know" the location of the fire panel, only the pull station. Drills during the facility's licensure year were not completed utilizing the alarm system approved by the local fire authority. Technical assistance was provided, regarding utilizing alarm system including possible methods of setting systems to "test mode" for the purposes of practice drills. Director was advised to locate alarm panel and contact alarm company for more specific and individualized instruction for drill practices. At least [1] fire drill utilizing the alarm system must be completed by the issued due date. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: Licensing monitor observed no change to slide structure. The top section of stairs on the large slide is disconnected from the slide structure. At time of inspection, Director called program owner to discuss reinspection. Owner stated that parts for slide repair were on back order at Home Depot. Licensing monitor requested verification that parts were ordered prior to non-compliance due date. Provider emailed a receipt for rubber mulch, not parts for repair. Following progressive enforcement procedures, a Notice of Violation is issued. This is the second non-compliance of this standard. 3.10.2(E), Handbook: When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. Diaper changing occurred on a surface that was not impermeable. (Section 2.1(A) Health and Safety, Page 3) Toilet/Bath Facility [SR] Comments: At the time of inspection, licensing monitor observed the changing pad in the 1-2yr old class that was not impermeable. The changing pad was covered in a crib sheet. The surface underneath the crib sheet was not impermeable. The same is true of the changing pad in the infant room, however per provider, no infants are currently enrolled. Technical assistance: the bleach and water solution utilized to clean while children are in care should reach a pH of 10 or less. Other cleaning products, such as Lysol, are considered to be toxins and may not be utilized while children are in care. 26.
