    Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [1yr old children were observed inadequately attended in the hallway]. Comments: At the time of inspection, licensing monitor observed the following The [1yr old] teacher walked into the classroom, leaving four [1yr old] children seated in a multi-passenger stroller in the hallway. Child care personnel stated they had returned to the classroom to get a hat; however, teacher was observed grabbing the item from the back of the classroom, not in sight or sound of children in the hallway. Another [1yr old] teacher was down the hall and facing the opposite way, pushing a multi-passenger stroller with six [1yr old] children, unable to provide additional supervision. • Ratio observed as follows: Total attendance at inspection 122 infant, 2:5 infant, 3:9 infant, 2:6 1yr, 1:6 1yr, 1:4 1-2yr, 2:8 2-3yr, 2:10 2-3yr, 2:13 3-4yr, 2:8 3-4yr, 2:10 4yr, 2:11 4yr, 2:12, and 1 RBT 4-5yr, 2:9 Source: FL DCF CARES inspection 2025-04-07.     3.9.5(B), Handbook: Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. 3.9.6(B), Handbook: The provider must make sure all formula and food brought from home are labeled with the childs first and last name. The provider is responsible for the label; therefore, if the label is not completed by the parent, the facility staff must put the label on when the formula or food is received. Bottles, sippy cups or food were not labeled and/or stored properly. Comments: At the time of inspection, licensing monitor observed several sippy cups and bottles from home in all classrooms that were not labeled or did not have first and last name.
