2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must include: (1.) Each childs name, (2.) The date and time of departure, (3.) Time of arrival at the destination, (4.) The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and (5.) The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). The facilitys log did not include all required information. (Section 2.1(A) Health and Safety, Page 3) Transportation [SR] Comments: The transportation logs were observed missing signatures and dates and times. Technical assistance was provided how to maintain logs as required. LICENSE/CERTIFICATES Compliance Comments The providers license expires on 3/31/2026. All areas of this report that fail to comply must be corrected by Friday, March 6th or the renewal license may be denied. During today's visit the facility was remeasured including the newly added building under the 20 square feet per child rule. The expected new license capacity is 204. Due to the number of toilets, the facility will be limited to an overall capacity of 200. Capacity change will be approved to the facility during the license renewal period. Technical Assistance: was provided to update the evacuation plan with the new building. Post-up-to-date capacity signs in all classrooms. 45. 21. Outdoor Play Space Group: Sec. 7-37, Ordinance and DCF Handbook (21-11)

3.5(F), Handbook: The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level, and be free from erosion or build-up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply and two exits must be provided. The facilitys fencing was inadequate in that [see comment]. Indoor/Outdoor Space [SR] Comments: The outdoor play area fencing was observed bent and detached in the corner of the playground. The Director stated during evening hours a truck was observed on the facility's camera crashing into the fence. The area was observed roped off and a work order is in progress to complete repairs. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: Remove large exposed roots from tree causing a tripping hazard. Repairs areas of grass turf with holes. Remove i table and chairs. Replace rusty bolts on base of blue slide, yellow car and red poles. Remove or replace blue tape on pole.
