05. Staff Ratio (Articles VIII.B, C, and XVIII.A, B) (05-01)

A staffing ratio of [1:4 ] for the children age [0-24 months ] is the minimum required. There was/were [1] staff for [5] children aged [0- 24 months ] observed. General Requirements [SR] Comments: During the inspection, observed there were five children to one staff in the infant room 0-12 months. There were also already 4 infants to one staff in the 12-24 months room. Staff moved additional child in younger infant room to older infant room, resolving non-compliance during inspection. Technical assistance was provided that staffing ratios with children must be maintained at all times for each age/classroom group. 34. Daily Attendance Log (Article XV.E) (34-01)

The daily attendance log for the [VPK] group was not completed to give real time information. Safety [SR] Comments: During the inspection, observed not all children were signed out on the daily attendance log for the VPK class. The staff for the VPK class promptly updated and corrected the attendance log for the classroom. Technical assistance was provided that all daily attendance logs must be complete to give real time information for each classroom/group. Safety glass was not used in the [bathroom of the VPK room ]. Comments: During the inspection, observed safety glass was not installed for protecting the light bulbs in the VPK class bathroom. Technical assistance was provided that all light fixtures must have safety glass covering to protect from the bulbs breaking/posing a hazard. The facility did not maintain a minimum distance of 18 inches around individual napping spaces. Safety [SR] Comments: During the inspection, observed three children in the 3- and 4-year-olds classroom group were napping on cots placed less than 18 inches apart. Director was informed and promptly corrected the violation by moving the middle child and their cot to another area with adequate spacing in the classroom, resolving the non-compliance at time of the inspection.Technical assistance was provided that all children must be maintained a minimum of 18 inches around individual napping spaces.
