   The outdoor play space was not adequately enclosed; fencing was missing, broken or not safely secure as evidenced by [gate in the outdoor play area wide open]. Physical Facility [SR] Comments: During the facility inspection, the gate in the outdoor play area leading to an unprotected area (the street) was observed to be wide open, creating serious safety and security concerns. This open gate could lead to unauthorized access to the outdoor area and other facility areas from unauthorized individuals, particularly concerning as children were present in the playground at the time of inspection. Additionally, the open gate creates potential for children to exit the facility unsupervised and access the street, posing significant safety risks. Technical assistance was provided to facility staff regarding proper perimeter security and gate maintenance requirements. The violation was corrected on-site when it was determined that the bolt of the gate was not holding properly and could not stay shut. The facility replaced the defective bolt and the gate can now remain securely closed. The facility shall ensure that all gates providing access to unprotected areas remain securely closed and latched at all times, particularly when children are present in outdoor play areas, to maintain proper perimeter security and prevent unauthorized access or child egress from the facility. 26.
