Children using the toilet/lavatory/bathing facility were not supervised. Comments: According to the video footage, on 4/22/2025, at approximately 11:25:24 student enters the classroom from outside, walks to the bathroom doors, enters the bathroom, light comes on. Teacher is at the door, another student enters the classroom, then run out of the classroom onto the outdoors area. At approximately 11:26:24 teacher moves from blocking the opened door and no longer in the doorway. Student exits bathroom at approximately 11:28:13 close the bathroom door and walks to the glass exit door to the playground and appears to be looking out. At approximately 11:29:47 a teacher opens the door and student exists out. At approximately 11:29:55, 2 teachers walk inside the classroom look inside the classroom.
