25. Outdoor Play Area Free of Debris/Fenced/Shaded (Article X.B) (25-02)

The facility's outdoor play area was observed to have [pavers/bricks, lose cable/cords, a tree stump, and short wooden pole ] which is hazardous. Physical Facility [SR] Comments: During the inspection, observed pavers/bricks loosely strewn by yellow wall of the back area of outdoor space. Facility had removed these items from outdoor area, resolving the non-compliance during the inspection. Additionally, observed one black and one white cable accessible to children in the outdoor area, which was mounted and taut as a correction. Observed tree stump accessible to children on outdoor area which was also removed/corrected. Observed a wooden pole with a white plastic tie around it, which was removed as well during the inspection. Non-compliances for hazardous items in outdoor area were ultimately all resolved while inspection was conducted. The facility's outdoor play area was observed to have [a hanging wire coming off the wall of the outdoor area ] which is hazardous. (Section 10.7, number 1) Physical Facility [SR] Comments: At the time of the inspection, observed a white wire going up the wall of the outdoor area that was not properly mounted. The wire was hanging and poses a hazard to children. Technical assistance was provided that the wire must be properly mounted to avoid posing hazards to children. The facility had no system for maintaining the sanitary condition of the equipment. Comments: During the inspection, observed the outdoor play area was not maintained. Play equipment was evidently not used daily, by the manner to which the equipment was stacked as if being stored, along the fencing. The outdoor area ground was mostly dirt and leaves, and the equipment stored by the fence was evidently not assembled, maintained, or frequently used. Technical assistance was provided that the sanitary condition of the equipment and use of equipment must be properly maintained in order for the children to play outside. 32. Child(ren) released to parent/guardian was/were not signed out by an authorized person. Comments: During the inspection, observed on the sign out sheet that parents are not signing out their children, but rather the staff. Staff explained parents sign waivers to have staff sign out the children on their behalf which was located in the children's files. Technical assistance was provided that children must be signed out by their parents, guardian, or the authorized person who picks up the child, regardless of the facility's waiver signed by parents. Article XV.1.a. "Daily sign in and sign out of children shall be recorded by the childcare facility personnel or the person who drops off the child, documenting the time when each child enters the facility or program. The custodial parent, guardian, or the authorized person who picks up the child shall be required to sign the child out, documenting the time the child was picked up. Persons authorized to pick up the child should be 16 years of age or older in keeping with Article XV(B)(4) of these rules." 35. 08. Daily Program (Article IX.A) (08-02)

The center failed to have a written plan of activities for the [VPK ] group of children. General Requirements [SR] Comments: At the time of the inspection, the inspector asked how often the children come outside to the play area. The director said the VPK children come out to play about once a week, and that the space is barely used. Technical assistance was given based on the following from rules and regulations: "ARTICLE IX. DAILY PROGRAM AND DISCIPLINE A. Daily program. 1. There must be a written daily program of scheduled activities that to provide meaningful age-appropriate experiences for the children. The scheduled activities should be designed and conducted to promote emotional, social, intellectual and physical growth; include quiet and active play, both indoors and outdoors; and include meals, snacks and nap times, if appropriate for the age and the times that the children are in care. 2. The activity program for both infants and other children must be conspicuously displayed in the childcare facility Providers are encouraged to advise parents or guardians of their childs activities on a daily basis, and to allow the parents or guardians to participate in the programs activities. 3. The program for infants must include adequate opportunity for activities for at least two (2) hours per day outside of the crib or playpen. A clean, safe, and appropriate area must be provided for this activity.
