(Section 10.4, number 4) Safety [SR] Comments: During the complaint inspection, observed video footage provided by the quality assurance director of the teacher in the Caterpillars class lifting a child by the arm inappropriately from napping on the cot. The cot was placed between the classroom child-sized table and the childrens bookshelf, likely leading to the teacher being unable to assist the child properly up from nap. This resulted in a hazard. Staff responsible for children in the outdoor play area failed to situate themselves so that all children could be observed and supervised. (Section 5, number 4) General Requirements [SR] Comments: The video footage captured at the facility shows several children playing on a large blue play equipment with slides. The first child in the video clip was observed walking down the slide. A few seconds later, the child (victim) and another male child wearing a gold/yellow shirt came down the slide. The child (victim) was the first in the group to come down the slide. He appears to hop or jump into the sliding position as he slid down the slide quickly. The male child behind him, began to slide down the slide and abruptly turned backwards on his way down. Once the boys reached the bottom of the slide, they returned to slide again. This time, the child (victim) moving quickly with his arms raised as if he was reaching to the bar, turn to the side as if he was attempting to slide down the slide from its edge or sit on the edge of the slide while waiting for his friends to arrive. The child (victim) then lost his balance and fell backwards to the ground. Shortly after, the male child wearing the gold/yellow shirt came down the slide backward and lying on his stomach. Once the child reached the bottom of the slide, he noticed the child victim on the ground. The child in the gold shirt was observed talking with the victim very briefly before leaving and returning with an adult. Several other children were observed continuing to slide down the slide to where the victim had been lying. The victim was observed attempting to get up or crawl away from the slide but could not. The adult did not appear in the video footage until coming to the childs assistance. From the time of student N.G fell from the slide to the ground to the teacher coming to his aid, it took 44.30 seconds. The dimension of the slide is approximately 5 ft and 10 inches (height) and 1 ft and 7 inches (width) and is helical in shape. This was resolved at the time of the inspection, as the incident occurred on 10/16/2026. 07. The facility failed to provide a ground cover or other protective surface under the [playground equipment] to reduce the incidence of injuries to children in the event of falls. (Section 11.2, number 3) Safety [SR] Comments: Per the director of the facility, mulch is under the turf surrounding the play equipment. However, the turf covering the area where student N.G fell was removed. Approximately 9 inches of compact dirt was measured under the turf. When other areas of the playground under said play equipment were observed, mulch was mixed with the dirt. No specifications of the fall zone were presented to the Environmental Specialist II. The child safety alarm device on a vehicle regularly used to transport children was not maintained in working condition. Comments: At the time of the inspection, during several attempts it could not be demonstrated to the Environment Specialist II that the alarm on the yellow school bus (tag number JQ373Q) was functioning properly. It is suggested that the driver of the yellow bus be orientated in knowing how to activate and deactivate the alarm on this bus. Van (tag number IW14GK) was monitored, and its alarm was functional.
