CCF Handbook, Section 7.1 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: During the time of the inspection, the Licensing Specialist observed 1 childs immunization record did not show completion or expiration date. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A -1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and Guidelines- Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. 42. CCF Handbook, Section 8.2, C Enforcement [SR] Comments: During the time of the inspection, the Licensing Specialist observed security camera footage which shows a staff member picking up a crying child and placing them aggressively on a cushioned chair. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. The facilitys storage of harmful items including cleaning supplies, flammable products, and poisonous, toxic and hazardous materials allowed access by children in care. CCF Handbook, Section 3.2, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of the inspection, the Licensing Specialist observed a staff bathroom in a hallway leading to the 2-year old classroom was unlocked. In the unlocked bathroom was an unlocked sink cabinet that left cleaning supplies and rubbing alcohol accessible to the children in care. A sink in the classroom was left unlocked which left cleaning supplies accessible to the children in care. During the time of the inspection, this was resolved by the Operator locking all cabinets that stored cleaning supplies. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook: B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times." 14.
