   The facility failed to maintain a current Employee/Contractor Roster for all child care personnel in the Clearinghouse. CCF Handbook, Section 5.1, K Comments: Licensing Specialist observed that the employee that was hired on 6/9/2025 was not listed or added on the Employee/Contractor Roster for all childcare personnel in the Clearinghouse within the required time frame. TA was provided: Licensing Specialist discussed requirement with the staff responsible. Staff Responsible added above mentioned to the Employee/Contractor Roster for all childcare personnel in the Clearinghouse during this inspection, thus returning this standard into compliance at the time of this inspection. CCF Handbook, Section 5.1,K. The employer/owner/operator must add childcare personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a childcare eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination.    47. Access/Child Safety CCF Handbook, Section 8 (47-01)

   Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Comments: Childcare personnel rubbed the child arm for three straight minutes using wet paper towel, the rubbing caused a large mark that was diagnosed by the doctor as a second-degree burn. Teacher was removed from classroom and the facility terminated the Childcare personnel immediately after the child's mother was contacted, thus returning this standard into compliance during the time of this inspection. Facility Contacted Licensing Specialist after contacted the child's parent, Licensing Specialist provided technical assistant, provider called the incident into the Hotline. CCF Handbook, Section 8.2, C: Childcare personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited. Owner/Director/Staff Responsible Comments We appreciate the opportunity to respond to the reported Level 2 violation. However, we respectfully and firmly disagree with the determination that this incident constitutes a Level 2 infraction. The safety and well-being of the children in our care is our highest priority, and we treat any incident involving a child with the utmost seriousness. In this instance, our center followed all established safety and reporting procedures. After a biting incident, the teacher promptly administered first aid and suspected that the childs reaction may have been due to the paper towel used. The teacher immediately informed the administrative team, who in turn contacted the childs parent, provided a detailed incident report, and shared photographs to ensure complete transparency. While the teacher acted in good faith with the intention of supporting and comforting the child, the parent expressed significant concern about the staff members actions. Out of respect for the familys concerns and to uphold our standards of care and trust, we made the decision to terminate the teachers employment. We believe the teacher acted with genuine care, but we prioritized the parents trust in our decision-making. We remain steadfast in our commitment to providing a safe, nurturing environment and ensuring that all families feel confident in the care we deliver. To reinforce this commitment, we are conducting additional staff training and reviewing our internal protocols, particularly around incident response and communication.
