25. Meals and Snacks CCF Handbook, Section 3.9 (25-02)

Children were provided food that was not age appropriate and/or posed a choking hazard. CCF Handbook, Section 3.9.3, B & C Comments: During the inspection, the Operator advised that the facility provides meals and snacks to the children that are in care. ***The provider is a food program participant. **** During the inspection, the Licensing Specialist observed that the lunch being servedbread and green beanshad not been cut into age- appropriate sizes before being given to the children in care. During the inspection, this has been resolved. The teacher cut the bread and green beans into half-inch pieces for the one-year-old children and quarter-inch pieces for the infant. During the inspection, the Licensing Specialist provided Technical Assistance (TA) per CCF Handbook: 3.9.3 Food Safety Handling of food in a safe and careful manner prevents the spread of bacteria, viruses and fungi. Outbreaks of foodborne illnesses have occurred in many settings, including child care facilities. C. Foods that are associated with young childrens choking incidents must not be served to children under 4 years of age, such as, but not limited to, whole/round hot dogs, popcorn, chips, pretzel nuggets, whole grapes, nuts, cheese cubes/sticks and any food that is of similar shape and size of the trachea/windpipe. Food for infants must be cut into pieces 1/4 inch or smaller, food for toddlers must be cut into pieces 1/2 inch or smaller to prevent choking. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the inspection, the Licensing Specialist observed broken area on the wall (under the toilet paper dispenser) in adult restroom which needs to be repaired. During the inspection, the Licensing Specialist observed water leakage signs on the ceiling tiles in the child restroom which require replacement. During the inspection, the Licensing Specialist provided TA per CCF handbook "3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months." The facility did not have an operable corded telephone available to all child care personnel during hours of operation. CCF Handbook, Section 3.8.1 Comments: During the inspection, the Licensing Specialist observed that there was no operable corded telephone available in the facility. During the inspection, the Licensing Specialist provided TA per CCF Handbook "3.8.1 Operable Phone There must be at least one operable corded telephone that is neither locked nor located at a pay station that is available to all child care personnel at all times during the hours of operation." During the inspection, the Licensing Specialist provided TA per CCF handbook "3.10.2 Diapering Requirements G. Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall they be placed on the diaper changing table." During the inspection, this has been resolved. CCF Handbook, Section 6.5, H Comments: During the inspection, the Licensing Specialist observed that one diaper cream has been expired. During the inspection, the Licensing Specialist provided TA per CCF handbook "6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: H. Medication that has expired or that is no longer being dispensed must be returned to the custodial parent or legal guardian or discarded." During the inspection, this has been resolved. 41. Immunization Records CCF Handbook, Section 7.1 (41-02)

The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: During the inspection, 2 of the children's records (who have been enrolled for more than 30 days and were on-site) did not include a current DH form 680, Florida Certification of Immunization in their files, as the immunization records had expired. During the inspection, Licensing provided technical assistance: Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. C. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. F. It is recommended that child care personnel are current with all immunizations routinely recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C-22.001(7)(v), F.A.C." 42. Student Health and Records CCF Handbook, Section 7.2 (42-02)

The Student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. CCF Handbook, Section 7.2 Comments: During the inspection, 1 of the children's records (who have been enrolled for more than 30 days and were on-site) did not include a current/valid Student Health Examination/DH Form 3040 or equivalent, as it has expired. During the inspection, Licensing provided technical assistance: Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. D. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. E. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. F. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. G. Any child who has or is at an increased risk for a chronic physical, developmental, behavioral or emotional condition and requires additional services must have a current Emergency Care Plan, prepared by the parent/guardian or physician, included in the childs file and readily accessible for those caring for the child. Child care personnel caring for a child with an Emergency Care Plan must be trained to recognize and respond appropriately to a medical emergency. Compliance Comments During the inspection, the Operator advised that there are currently 12 children enrolled. During the inspection, Licensing monitored 12 of the 12 children records.
