   A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [9] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: 1:5 Mixed (Four and Five-year-olds) 1:9 Mixed (3 One-year-olds, and 6 Two-year-olds) (Out of compliance) 1 Staff Second Ratio After Standard was brought back into compliance: 1:5 Mixed (Four and Five-year-olds) 1:5 Mixed (3 One-year-olds, and 2 Two-year-olds) 1:4 Two-year-olds 1 Staff During the complaint inspection, the facility was found out of compliance for ratio, in that in the one-year-old classroom, there were 3 one- year-olds, 6 two-year-olds with 1 personnel. The standard was brought back into compliance in that a second staff member came in the classroom. This standard was brought back into compliance in that another staff member came into the classroom while the licensing specialist was still in the classroom changing the ratio from 1:9 (3 one and 6 two-year-olds) to 2:9. The first staff member moved 4 children to a separate classroom. Updated ratio: 1:4 (Two-year-olds), the second staff member remained in the classroom: Ratio: 1:5 (3 One-year-olds and 2 Two-year-olds) During the inspection, the Licensing Specialist provided technical assistance, s.402.305(4), F.S. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed childcare facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one childcare personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one childcare personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one childcare personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one childcare personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one childcare personnel for every 20 children. 6. For children 5 years of age or older, there must be one childcare personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. (b) This subsection does not apply to nonpublic schools and their integral programs as defined in s. 402.3025(2)(d)1. In addition, an individual participating in a community service program activity under s. 445.024(1)(e), or a work experience activity under s. 445.024(1)(f), at a childcare facility may not be considered in calculating the staff-to- children ratio.    Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D Health Requirements [SR] Comments: During the complaint inspection, the Operator provided the Licensing Specialist with 10 accident/incident reports. Of the 10 accident/incident reports, 4 reports were not signed by the individual authorized to pick up the child on the date of the accident/incident. The Operator has been given a due date of 11/3/2025 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 6.4 B Accident/Incident Notification - B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.
