The facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Licensing Specialist observed the following; 1. Ones and Twos playground: All the plastic purple climbing stones on the beige climbing walls connected to the green slide equipment were dry and had broken edges, , and the blue platform connected the same equipment had hole. 2. Preschool/School age Playground: The yellow metal slide had a rusty spot on the bottom area where children lands. TA: Licensig Spesialist discussed requirement with the provider: All climbing stones must be replaced, the platform hole must be repaired appropriately and the rusted ary must be sanded down and repainted. Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. CCF Handbook, Section 4.3, B (Section 2.1 Health and Safety, Page 3) Training [SR] Comments: Licensing Specialist observed a Child Care personal, who was not in compliance with training requirements when she left the industry, did not complete required training and any new mandated training before returning to the industry. Above mentioned employee did not finish introductory training before leaving the industry. Employees industry start date was 02/20/2023, employee left industry on 07/18/2024 and was not incompliance with her training. This employee was hired by this facility on 04/23/2025. TA was provided: Licensing Specialist discussed requirement with the provider. CCF Handbook, Section 4.3 B, Break in Service A. In the event an individual leaves the child care industry in compliance with training requirements and returns to the industry either at the same or a different child care facility, he or she will be given 90 days to comply with any new training requirements established during the gap in employment in the child care industry. B. In the event an individual leaves the child care industry not in compliance with the training requirements and returns to the industry either at the same or a different child care facility, he or she must comply with the training requirements described in this section, as well as any new training requirements that may have been added during the gap in employment in the child care industry prior to re- employment. 03. Ratio Sufficient CCF Handbook, Section 2.3 (03-01)

A ratio of [2] child care personnel for [6] children is required. A ratio of [1] child care personnel for [6] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: During this inspection Licensing Specialist observe a ratio of 1 Childcare Personnel to 6 Children, Ages: infants and ones. Director informed licensing Specialist that she was in the class, but she had to step out to help a parent. TA was provided: Director stepped in the class and called another childcare personal to come to the school and help with the ratio. During the 2nd walk through Licensing Specialist observed the following Ratios: 2: 6, Three infants and Three 1 Year Old. 1:9 Two-Year-Old. 1:12 Three- to Five-Year-Old. Thus, returning this standard into compliance at the time of this inspection. Child Care Facility Handbook 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. s.402.305(4), F.S.(4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 (Section 2.1 Health and Safety, Page 3) Comments: Licensing Specialist observed that Childcare personal who work 10 hours or more per month did not complete the 40-hour Introductory Child Care Training requirement. Above mentioned personal was missing Child Care Facility Rules and Regulations (RNRF) TA was provided: Licensing Specialist discussed requirement with the director, Employee registered for the course and enrolled in the competency exam during this visit. The Exam is scheduled for 07/10/2025. CCF Handbook Section 4.2.1 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. Part I Courses (30 Hours) Child care personnel must complete all of the following: Child Care Facility Rules and Regulations • Health, Safety and Nutrition • Identifying and Reporting Child Abuse and Neglect • Child Growth and Development • Behavioral Observation and Screening • B. Part II Courses (10 Hours) Child care personnel must also complete 10 hours of the following Part II courses: Special Needs Appropriate Practices (10 hours), or • Understanding Developmentally Appropriate Practices (5 hours) and one • of the following courses: 1. The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C (Section 2.1 Health and Safety, Page 3) Comments: Licensing Specialist observed that the facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course 2 childcare personnel .The first childcare personal started working on 01/16/2024 with an industry start date of 01/16/2024, and the second childcare personal started working on 03/31/2025 with an industry start date of 03/07/2022 within 12 months of date of employment in child care industry. TA was provided: Licensing Specialist discussed requirement with the director. Above mentioned employees were immediately enrolled in the Department approved five-hour early literacy and language development course (ELC). 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C.
