A vehicle(s) used to transport children did not have the required alarm system installed to prompt the driver to inspect the vehicle for children before exiting the vehicle. CCF Handbook, Section 2.5.4 (Section 2.1 Health and Safety, Page 3) General Requirements [SR] Comments: During the time of inspection, Licensing Specialist observed the vehicle alarm was located at the front of both facility vehicles. 2.5.4 Vehicle Requirements e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 08. Indoor and outdoor play areas were not inspected daily for basic health and safety and/or any problems were not corrected before the play area is used by children and/or daily inspection logs were not maintained for 12 months. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, Licensing Specialist observed the wooden playhouse in the outdoor play area to be covered in cocoons. Licensing Specialist observed fuzzy caterpillars crawling on the roof and railings of the playhouse. Licensing Specialist observed Director brush the cocoons and caterpillars off of the playhouse. This standard was brought back into compliance during the time of inspection. CCF Handbook, section 3.1 General Health and Safety Requirements A. CCF Handbook, section 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, Licensing Specialist observed holes with sharp edges in the white fence on the older children's playground. The ground cover or other protective surface under the [slide, swings, climbing play equipment] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of inspection, Licensing Specialist observed that the mulch on the toddler playground measured less than an inch. The mulch on the older children's playground measured 3 inches or less in areas around the slides and swings. CCF Handbook, section 3.12 Outdoor Equipment D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area.
