An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the time of the inspection the Licensing Specialist observed 2 ceiling tiles in the Lunchroom area that have some damage. 1 has some water damage and the other is lifting from the frame in the ceiling. Per the DCF Handbook All child care facilities must be clean, in good repair, free from health and safetyhazards and from evidence of, or presence of, vermin infestation. Indoor play areasmust be inspected daily for basic health and safety and documented on a dailyinspection log. Outdoor play areas must be inspected daily for basic health and safety.Any problems must be corrected before the play area is used by children.Documentation of the indoor play area inspection must be maintained for 12 months. CCF Handbook, Section 3.12, B and F Comments: During the time of the inspection the Licensing Specialist observed that the blue plastic bench attached to the play gym has a crack on both sides of the bench located on the preschool playground. The provider was provided extra time. Per the DCF Handbook All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. CCF Handbook, Section 7.1 Comments: During the time of the Inspection the Licensing Specialist observed 1 child file with an expired immunization record. Per the DCF Handbook The child care facility is responsible for obtaining for each child in care a current, completeand properly executed Florida Certification of Immunization form Part A-1, B, or C, DH680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the ReligiousExemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DHForm 681 may be obtained from the local county health department. Specific immunizationrequirements are included and detailed in the most current edition of the ImmunizationGuidelines-Florida Schools, Child Care Facilities and Family Day Care Homes aspromulgated by the Florida Department of Health.A. Immunizations received out-of-state are acceptable; however, immunizations must bedocumented on the Florida Certification of Immunization form and must be signed bya physician practicing in the State of Florida.B. If the custodial parents or legal guardians fail to provide the documentation requiredabove within 30 days of enrollment, the facility shall not allow the child to remain in theprogram.
