Areas of the facility were found to be in need of cleaning. CCF Handbook, Section 3.1, A Comments: During the time of the inspection, the Licensing Specialist observed in the shared bathroom in room 3, under the right sink there is a large green mold spot. The changing table in the shared bathroom has dirt and build up under it and around the edges. In room 5 under the sink there is a large mold spot. Roach droppings were observed on the windowsill in room7. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:A. All child care facilities must be clean, in good repair, free from health and safetyhazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the time of the inspection, the Licensing Specialist observed two holes in the walls of room 7, one in the hallway where the bathrooms are and the other on the wall in the classroom. In room 7 on the closet doorknob is loose and coming out of the door, under the doorknob there is a hole with peeling paint around it. In room 7 the rubber trim in the right side bathroom is peeling off of the wall. In room 2 there is a piece of the blinds that is broken and being held together by tape, there is a stained ceiling tile in the corner by the exit door. There was evidence or the presence of rodent or vermin observed in the facility. CCF Handbook, Section 3.1, A Comments: During the time of the inspection, the Licensing Specialist observed several roaches (dead and alive) under the sinks in rooms 3, 4, 5, 6, and behind a filing cabinet in the HS1 room. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:A. During the facility's operating hours, an activity occurred in a portion of the building which endangered the health and/or safety of children in care. CCF Handbook, Section 3.1 Comments: During the time of the inspection, the Licensing Specialist observed A/C maintenance being conducted while children were in care. A repair man was observed on a ladder with a drill on the ramp to the EHS building replacing A/C filters. During the time of the inspection this was resolved by the repair man completing the job and leaving the facility.During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:A. All child care facilities must be clean, in good repair, free from health and safetyhazards and from evidence of, or presence of, vermin infestation. Indoor play areasmust be inspected daily for basic health and safety and documented on a dailyinspection log. Outdoor play areas must be inspected daily for basic health and safety.Any problems must be corrected before the play area is used by children.Documentation of the indoor play area inspection must be maintained for 12 months.B. It is the responsibility of the director/operator to ensure all areas and equipment of thefacility are free from fire hazards, such as lint build up in heating and air vents, filters,exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens,stoves and food equipment.C. Animals must be properly vaccinated (if a vaccination is available for the specificanimal), free from disease, and clean. Animals that are poisonous, venomous,aggressive, or pose a potential threat of harm to children in care are prohibited.Parents/guardians must be informed in writing of all animals on the premises. Notice may be provided by a conspicuously posted notice or bulletin, policy handbook, parentflier, or a statement included on the enrollment form. Current vaccinations recordsmust be available for review by the licensing authority. No animal may freely roam theindoor/outdoor premises. Animals are prohibited in areas where food is prepared,stored, or served. If animals or birds are kept in classrooms, they shall be caged andcages must be kept clean.D. Strangulation hazards, strings, and cords long enough to encircle a childs neck suchas those on toys and window coverings must not be accessible to children in care.E. Pursuant to Chapter 386.204, F.S., smoking is prohibited within the child care facilityand in vehicles when being used to transport children. Smoking is prohibited in alloutdoor areas, including on field trips, while children are in care. Owners/operators areto notify custodial parents and legal guardians, in writing, that smoking, including ecigarettesand vaping, is prohibited on the premises of the child care facility.F. Design and construction of a new child care facility or modifications to an existingfacility must meet the requirements of the applicable local governing body. The facilitymust provide current written approval from the local governing body to verifycompliance with building requirements, which include construction of a new building;renovation of an older building; or after a natural disaster to properly evaluate andwhere necessary, remediate or avoid sites wheres childrens health could becompromised. The written approval must include assessments of:1. Potential air, soil, and water contamination on facility site and outdoor play areas;2. Potential toxic or hazardous materials in building construction, such as lead andasbestos; and3. Potential safety hazards in the community surrounding the site.G. Cleaning must not take place while rooms are occupied by children except for generalclean-up activities that are part of the daily routine. General cleaning refers to cleaningnecessary to maintain a sanitary environment but that does not pose a hazard tochildren, such as wiping the table after lunch, soaking toys in a tub on the countertop,or sweeping. This does not include cleaning with hazardous materials or any cleaningwhich poses a risk of slipping or falling.H. Pest control must not take place while rooms are occupied by children. The play equipment was not maintained in a safe or sanitary condition for the children to use in that [the play caterpillar in the HS1 play area has cracks down the middle of its back.]. CCF Handbook, Section 3.12, B and F Comments: During the time of the inspection, the Licensing Specialist observed the play caterpillar in the HS1 play area has cracks down the middle of its back. During the time of the inspection, the Licensing Specialist provided technical assistance, per the CCF Handbook:B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the time of the inspection, the Licensing Specialist observed dead roaches, droppings, and residue from water leaks under the sinks in the kitchen and rooms 3, 6, and 7. During the time of the inspection, the sink cabinets were cleaned by the janitorial staff. During the time of the inspection the Licensing Specialist observed the baseboard in room 7 peeling off of the wall exposing gap between baseboard and wall. During the time of the inspection, the Licensing Specialist observed a sink in room 7 was not operational. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "A. The facility did not have an operable corded telephone available to all child care personnel during hours of operation. The facility did not have documented proof of an annual fire inspection by the local fire authority. CCF Handbook, Section 3.8.2, A Comments: During the inspection, the Licensing Specialist monitored the Fire Drill Log and observed documentation of the last fire drill having been conducted on 2-13-25. The last approved fire inspection for this facility was dated 2-13-24. The fire extinguisher certifications are valid until 11-2025. During the time of the inspection, the Licensing Specialist provided technical assistance: per the CCF Handbook "A. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program."
