   The facility did not have documented proof of an annual fire inspection by the local fire authority. CCF Handbook, Section 3.8.2, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: Licensing specialist reviewed fire inspection on site and it was dated 5/7/2025. Provider stated they had already received an inspection and contacted fire department for a copy. Fire inspector told provider that they would send a copy on Monday, 6/1/2026, morning. Provider sent a copy of fire inspection on 6/2/2026 dated creation date of 6/1/2026 with a violation and corrected violation on 6/2/2026, which both occurred after licensing specialists inspection. Technical assistance was provided that To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program. Last fire drill was conducted on 5/1/2026. One fire drill using an alternate evacuation route was done on 5/28/2026. One fire drill during napping/sleeping times was done on 5/28/2026. One fire drill in the presence and at the request of the licensing authority was done on 5/28/2026. One emergency LOCKDOWN drill has not been conducted during licensure year. One emergency INCLEMENT WEATHER drill has not been conducted during licensure year. Fire inspection conducted on 5/7/2025. Fire extinguisher serviced on 04/2026.    A ratio of [1] child care personnel for [4] children is required. A ratio of [1] child care personnel for [6] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Upon arrival to the facility, licensing specialist observed through the door, a staff was cleaning in the hallway. Then she went into her classroom. Licensing specialist went in to that classroom when entering the school. The staff was part of the infants classroom where there were 6 children and two staff to maintain ratio must be in the classroom at all times. Licensing specialist also observed 8 children in the 1-year old classroom with one staff. A second staff returned to the classroom and said she had gone out to throw away the garbage. Technical assistance was provided that he staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B.
