DH Form 680 and DHForm 681 may be obtained from the local county health department. CCF Handbook, Section 3.6.4, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: At the time of inspection, the inspector observed an infant in the crib with a stuff animals on her stomach. Technical assistance was provided to the teacher to make sure any time there are infants inside of the cribs that there is not in there with them. The teacher removed the item, leaving the infant in the crib with just the fitted sheet. TA was given to the provider referencing the CCF Handbook. 7.4.1 Background Screening Documents Background screening documentation must be maintained for all childcare personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C.
