The facility did not have documented proof of an annual fire inspection by the local fire authority. Unless statutorily exempted, all childcare facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Record Keeping [SR] Comments: Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida. L.S observed that 1 child was missing their immunizations records, or immunizations records were expired. TA was given that in order to obtain compliance provider must have immunizations records on site ready for L.S review at the time of inspection.
