SACCF Handbook, Section 3.8.2, A Comments: The provider was contacted via email and asked to submit a copy of the current and approved fire inspection report issued by the local fire authority. However, the provider failed to submit the required requested documentation. As a result, the provider is unable to meet compliance requirements and will be placed on a second provisional license. This marks the providers third violation of this requirement, and a per day fine will be imposed until the issue is resolved.
