21. Fire Drills & Emergency Preparedness SACCF Handbook, Section 3.8 (21-06)

During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [October, November and December.]. SACCF Handbook, Section 3.8.4, A Comments: License Specialist observed that there was no documentation to show that during the facility's licensure year, fire drills utilizing the approved alarm system were not conducted monthly since the last Routine Inspection was conducted on 10/24/2024. Fire Drill for the month of January will also be required by the time of re-inspection due by 2/12/2025. Technical Assistance was given to the Provider as per the School Age Child Care Facility Handbook that: ( During the facilitys licensure year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. *** The operator must maintain and post in an easily seen location a written record of emergency preparedness drills showing the type of drill, date conducted, number of children and child care personnel in attendance, and time taken for all individuals to complete the drill. Documentation of conducted fire and emergency preparedness drills must be available at the time of inspection. Areas of the facility were found to be in need of cleaning. SACCF Handbook, Section 3.1, A Comments: Licensing Specialist observed several ceiling vents/fan areas of the facility were in need of cleaning. One in the boy's bathroom, located in the computer room , in the Lobby Area at the entrance of the Administrative Office and in the common areas. Technical Assistance was given to the Director as per the SACCF Handbook that ( All school-age program facilities must be clean, in good repair, and free from health and safety hazards and vermin. During the hours that the program is in operation, no portion of the building can be used for any activity that presents a risk to the health and safety of the children. Indoor and outdoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Any problems must be corrected before the play area is used by children. Documentation must be maintained for 12 months. It is the responsibility of the director/owner to ensure that all areas and equipment of the facility are free from fire hazards, such as lint and dust build-up in heating and air vents, filters, exhaust fans, ceiling fans and dryer vents. 12. Facility Environment SACCF Handbook, Section 3.1 (12-02)

An area(s) of the facility was observed to not be in good repair. SACCF Handbook, Section 3.1, A Comments: Licensing Specialist observed several areas of the facility were in need of repair. 1. There were several water spots on ceiling tiles in the Computer Room, Lobby/Entrance classroom, 2nd Classroom in front of the Food Preparation Area, and common areas/hallways throughout the facility. 2. Closet's Door Knob for A/C Unit, (located in the BIG Classroom in front of the Food Preparation Area) is broken, missing the inner lock making all contents accessible to the children in care. 3. The Air vent in the same room for the A/C unit mentioned above is loose and missing multiple screws making it a potential safety hazard for the children in care. 4. The Pantry Door in the Food Preparation Area is missing the lower door vent, leaving a large hole on the door with splinters and sharp edges to the children in care and also vulnerable to unwanted entrance to food stored when actual door is closed. 5. No electricity and exposed cables in the electrical outlet by the playground door's entrance/exit. License Specialist assured the exposed cables by adding protective cover with electrical covers and Director moving the book shelf in front of the outlet at the time of inspection to avoid accessibility by the children in care. 6. Boy's bathroom in the Computer Room: a. Contained peeling paint in the entire bathroom door. b. Multiple floor tiles were missing and broken at the entrance of the bathroom with sharp and jagged edges. c. The electrical outlet on top of the toilet which is across from the shower area, had no covers and did not meet GFCI (Ground Fault Circuit Interrupter) needed in areas of high moisture such as bathroom, kitchens etc. d. The BATH FACILITY/Shower in this bathroom is being used as storage. 7. The window in the Computer Room was missing all the interior sheetrock/Window sill, horizontal structure and surface at the bottom of the window, which serves to structurally support and hold the window in place. Refinishing needed for window structural support. The facility did not maintain the sink, toilet and/or bath facilities used by the children in good operating condition, clean and sanitized or disinfected, posing a threat to the health, safety or well-being of the children. SACCF Handbook, Section 3.7, E Comments: Licensing Specialist observed that there was no electricity/light nor a light fixture covering in the Bath/Shower/Bathroom located next to Director's office which S/A children, staff and guest use. Other Bath/shower available in the facility was being used as storage and the one available here is inaccessible since there is currently no light nor electricity. There was currently no shower/bath available for use at the time of inspection. SACCF Handbook, Section 3.9.2, B Comments: Licensing Specialist Observed a bag of fruit/apples in the refrigerator located in the Food Preparation Area. Label Stated: EZ Meals 2 Go Inc. 09/26 Step Up - 28 Fresh Apple Technical Assistance was given to the Director as per the SACCF Handbook that ( Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food- borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and not in bathroom facilities. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. A. Food containers, such as cans, plastic containers, boxes, and bags, must be stored above the floor on clean surfaces protected from splash and other contamination. B.
