CCF Handbook, Section 2.8, E General Requirements [SR] Comments: It was observed by LS that the disciplinary policy was not signed by (MV)'s parent. Technical assistance was given per CCF handbook "A copy of the current disciplinary and expulsion policies must be available for review by the parents or legal guardian and the licensing authority. Providers must have a comprehensive discipline policy that includes developmentally appropriate social-emotional and behavioral health promotion practices, as well as discipline and intervention procedures that provide specific guidance on what child care personnel should do to prevent and respond to challenging behaviors. Preventive and discipline practices should be used as learning opportunities to guide childrens appropriate behavioral development." Child care personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred. CCF Handbook, Section 6.4, B & D Health Requirements [SR] Comments: Based on statements provided, parent of (MV) was not given a copy of the incident/accident report. The incident/accident report was also observed by LS to not have the signature of parent/legal guardian. Technical assistance was offered per CCF handbook " B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.
