Regarding the unauthorized release of a child, from the aftercare program. Based on staff interviews and documentation obtained, it was determined that the facility released the child to an individual who was not authorized in writing by the custodial parent(s) or legal guardian(s). Based on staff interviews and documentation obtained, Specifically, documentation revealed that the parent attempted to provide written authorization via email identifying a designated individual approved for pick-up; however, due to reported internet issues at the facility, the updated authorization was not received at the time of release. Despite not having written authorization on file, the facility released the child to an individual whose name did not match the person identified by the parent. This indicates that the facility failed to verify the identity of the authorized individual prior to release, resulting in the child being released to an unknown and unauthorized individual. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual. Technical Assistance: Technical assistance was provided regarding CCF Handbook, Section 2.4.1(E). The facility was advised that it is responsible for the supervision of each child until the child is released to an authorized individual. Children must only be released to persons identified in writing by the custodial parent(s) or legal guardian(s), and authorization must be received and verified prior to release. The facility was further advised that all individuals picking up a child must present a valid photo identification, and staff must verify that the individual matches the authorized person listed in the childs records before releasing the child. Under no circumstances should a child be released without proper written authorization on file. Additionally, the facility was advised to implement procedures to ensure that authorization forms are received and confirmed prior to dismissal, including contingency plans for situations involving technical or internet issues. The facility was encouraged to maintain accurate and up-to-date records of authorized individuals and to ensure all staff are trained on proper release procedures to prevent unauthorized release of children. 05.
