General Requirements [SR] Comments: Licensing specialist arrived to the facility and observed through the glass door a staff was moving children from one class to another. Another staff arrived at the same time as licensing specialist, and all the children were moved to their respective classrooms. Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. The Student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. CCF Handbook, Section 7.2 Record Keeping [SR] Comments: Licensing specialist reviewed one child's file and observed two student health examinations in the file that are over two years old. A current one is not available. Technical assistance was provided that The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. The Clearinghouse roster list is shared among the facilities that have the same ownership and there are staff from other same ownership centers. However, there are staff who previously worked in this center and have not been removed. Technical assistance was provided that The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a child care eligible result and has been hired at the facility. Supervision of children in the [infants] group was inadequate in that [the teacher was away from the classroom]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: Licensing specialist arrived to the facility and upon entrance to the lobby area, observed one staff leave the kitchen/ dining area and return to the classroom where infants are present. Licensing specialist walked into the classroom after the staff and observed no other staff in the classroom and no other staff walked out. Technical assistance was provided that Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Staff returned to classroom immediately after licensing specialist's arrival and standard returned to compliance at time of inspection. The facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A Physical Environment [SR] Comments: Licensing specialist reviewed fire inspection for facility. Per director, a fire inspector visited the facility for an inspection, however, they did not finish the inspection due to something missing. Per director, a fire inspection report was not provided that time. Technical assistance was provided that To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program.
