The facility did not have documentation of an annual vehicle inspection for a vehicle used to transport children in care. CCF Handbook, Section 2.5.4, A General Requirements [SR] Comments: Licensing specialist was unable to review a current vehicle inspection. The one inside the van expired in January 2026. Per provider, she took the vehicle and has a sticker on the van, but the actual document was not available for review. Technical assistance was provided that All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. The facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A Physical Environment [SR] Comments: Licensing specialist reviewed on-site fire inspection and it was expired. Director was called by staff and the most recent fire inspection was e-mailed to licensing specialist, however, it is not approved as it has a Notice of Violations (N.O.V.) for results. Technical assistance was provided that To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector. A. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A- 36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. Attendance records did not include the time of each childs arrival and departure. CCF Handbook, Section 7.5 Record Keeping [SR] Comments: Licensing specialist reviewed attendance logs for children on-site. The 1-year old and school age classrooms did not have attendance with arrival and departure time completed for today. In the school age classroom (which was outside), started writing down the names of the children. After reviewing parents sign-in/sign-out books, not all children had a sign-in/sign-out page in the books and some were not signed in. Technical assistance was provided that Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. Technical assistance was provided that The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001 (7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Provider is awaiting fire inspection and called the fire department while licensing specialist was on-site. One emergency LOCKDOWN drill was done on 1/7/2025. One emergency INCLEMENT WEATHER drill was done on 2/10/2025. Fire inspection conducted on 2/29/2024. Fire extinguisher serviced on 10/2024. As technical assistance, the licensing counselor requested that a Fire Drill be done in the presence of licensing authority, and the provider agreed to conduct the fire drill at time of inspection. The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [17 children files were missing signed brochure]. (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing specialist reviewed 17 children files with no distracted adult brochure signed by parent during September 2024. Technical assistance was provided that annually, in the months of April and September, the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.CF/PI 175- 12, brochure, which is incorporated by reference in 65C-22.001(7)(x), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com . 18 of 74 childrens enrollment/health records were reviewed for compliance.
