An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: LS observed the ground by the fence was not in good repair. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. 32. Outdoor Equipment CCF Handbook, Section 3.12 (32-03)

The play equipment was not maintained in a safe or sanitary condition for the children to use in that [bicycles seats were broken and cracked]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] Comments: LS observed bicycle seats on playground were broken. Provider has resolved the non-compliance at the time of the inspection by removing the bicycles. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 (Section 2.1 Health and Safety, Page 3) Comments: While reviewing the personnel files, LS observed 1 personnel (YP) failed to complete the required 40-hour introductory child care training requirement. in which the personnel was missing Part One ( Health, Safety and Nutrition, ? Child Growth and Development ? Behavioral Observation and Screening) and Part 2, ( ? Special Needs Appropriate Practices (10 hours), or ? Understanding Developmentally Appropriate Practices (5 hours) and one of the following courses: 1. Infant and Toddler Appropriate Practices (5 hours) 2. Preschool Appropriate Practices (5 hours) 3. School- Age Appropriate Practices (5 hours)). Technical assistance was provided per CCF handbook " Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A.
