03. Ratio Sufficient CCF Handbook, Section 2.3 (03-01)

A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [7] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Licensing specialist observed 6 children in the 1-year old classroom and one school age child in the 1-year old bathroom. The school age child was taken to her class which staff said was kindergarten. Licensing specialist went to the other building where VPK and Kindergarten classes are and both classes were having lunch. Licensing specialist reviewed attendance record and the childs name did not appear and the staff did not know her last name. Licensing specialist requested to review file and a file was not on-site. Staff in charge was not present but communicated via telephone with licensing specialist and explained that the child is the child of a staff they are looking to hire and is in the process of completing enrollment paperwork for the child. Ratio at Time of Arrival: 1-Year Old - 1:7 2-Years Old - 1:7 2-Years Old - 1:9 3-Years Old - 1:12 VPK - 1:11 Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. Child care personnel failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 7.5, B (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing specialist reviewed children's attendance logs. The attendance logs used in the classrooms did not have arrival and departure times. The parents sign-in and sign-out logs were used but not all children present were signed in. Technical assistance was provided that daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.]. CCF Handbook, Section 7.3, C.5. Record Keeping [SR] Comments: During the time of inspection, the licensing specialist observed that the provider failed to maintain documentation verifying that the parent (s) or legal guardian(s) of each child were provided with the required information regarding the potential for distracted adults to unintentionally leave a child in a vehicle, as mandated to be distributed annually in the months of April and September. Per the Child Care Facility Handbook, Section 7.3, C.5, child care facilities must provide this information to parents/guardians to raise awareness of the risk that distracted adults may fail to drop off a child at the facility and instead leave the child in the vehicle. CCF Handbook, Section 7.4, C Record Keeping [SR] Comments: During the time of inspection, the licensing specialist observed that the personnel/volunteer record for an individual working ten hours or more per month did not include a current, signed CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form. As required by the Child Care Facility Handbook, Section 7.4, C, this form must be signed annually by all personnel and applicable volunteers to acknowledge their responsibility in reporting suspected child abuse and neglect.
