The facilitys fencing walls or gate area had gaps that could allow children to exit the outdoor play area. CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Upon inspection of the outdoor play area, the Licensing Specialist (LS) observed a gap between the fence gate and the adjoining fence section. The gap created a potential safety hazard, as it could allow children access outside the designated play area. TA was provided to the director regarding the need to secure and adjust the fence gate to eliminate the gap and ensure the outdoor area remains safe and secure for children in care. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. Child care personnel working in the food preparation area did not wear proper head covering. CCF Handbook, Section 3.9.1, B (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Comments: During the inspection, the Licensing Specialist observed the person serving food was not wearing hair nets or other appropriate hair coverings. The LS advised staff that hair nets or hair coverings must be worn at all times while preparing and serving food to ensure compliance with health and sanitation requirements. The issue was corrected during the inspection when the director provided the staff member with a hair net. Child care personnel, while working in the food preparation area, must wear proper head covering, such as a hair net or hat. To prevent contact with readyto- eat foods, child care personnel must use clean disposable gloves, utensils, or similar items in the food preparation area. The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [Distracted Adult Brochure not on children file sign by the parent/legal guardian during the month of April.]. CCF Handbook, Section 7.3, C.5. (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: (35 of 110) childrens enrollment/health records were reviewed for compliance. Files reviewed: Letters (M Z), and all newly enrolled children since the last inspection. Licensing Specialist reviewing the children file notice that not all children have on file sign by the parent/legal guardian the Distract Adult Brochure during the month of April. T.A. Child care personnel failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 7.5, B (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing Specialist reviewing child care attendance notice that not all children are in the attendance roster. Per provider the attendance roster was fix and include every children in the classroom at the time of the inspection. T.A. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program.
