25. Meals and Snacks CCF Handbook, Section 3.9 (25-07)

A refrigerator was observed without a thermometer or with a thermometer showing a temperature reading greater than 41 degrees Fahrenheit. CCF Handbook, Section 3.9.2, F.1. Comments: Licensing specialist observed the refrigerator temperature was at 46 F. The provider had maintenance come to lower the temperature; however, the refrigerator was not reducing the temperature. The food was moved to another refrigerator in the kitchen that had a temperature below 40F. Maintenance removed the broken refrigerator for repair. The issue was resolved at the time of inspection. Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. 17. Outdoor Play Area/Fencing CCF Handbook, Section 3.5 (17-02)

The facility's outdoor play area contained litter, nails, glass or other hazards that posed a threat to the health, safety or well-being of the children. CCF Handbook, Section 3.5, A Comments: 3.12 Outdoor Equipment D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. 2. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply with ASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. Playground Safety Observation Report: "During the inspection, while children were playing in the playground, the following concerns were noted:" Fungal Growth: The inspector observed the presence of five (5) mushrooms growing on the playground surface. The presence of mushrooms may indicate excess moisture and poses a potential health risk to children. • Inadequate Protective Surfacing: It was also observed that the playground lacks appropriate protective surfacing such as mulch, rubber, or sand, which is required to minimize injury from falls and to comply with standard playground safety regulations. • Technical Assistance: Technical guidance was provided, advising that staff conduct a daily inspection of the playground each morning prior to allowing children to access the area, to ensure it is safe and free from hazards. Program Response: The center director reported that they are currently awaiting a quote for mulch replacement and will proceed with refilling the surfacing once the quote has been received and approved. 18. Bedding and Linens CCF Handbook, Section 3.6 (18-12)

Bedding and/or linens were not stored in a sanitary manner. CCF Handbook, Section 3.6.1, G & H Comments: Licensing specialist observed beddings stored on the cots stacked on top of the other. T.A. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. 33. Training Requirements CCF Handbook, Section 4 (33-01)

Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 Comments: Licensing Specialist when reviewing the child care personnel file found that the child care personnel does not complete the 40-hour Introductory Child Care Training requirement. Licensing Specialist give the child care personnel an extended time frame due to lack of available date for texting. T.A. Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better.
