The facility did not have 35 square feet of usable floor space per child for the number of children observed in care. The facility had a capacity of [10] based on 35 square feet and [45] children were observed in care. The facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: The inspector observed that the provider has a fire inspection report with two violations that have not been corrected and are now past due. TA The provider must ensure that all violations cited in the fire inspection report are corrected promptly. Fire safety inspections must remain in compliance at all times, as emphasized by the inspector, to ensure the safety of all occupants. In the form of efforts to assist licensees, the next violation of above standard within the next two years of last violation can result in disciplinary sanctions being progressively enforced by the Department imposing a fine in accordance to section 402.310 Florida Statues and Rule 65C-22.010, Florida Administrative Code. Records were not available for review by the Licensing Authority at the time of this Renewal Inspection. Technical Assistance was given to the Provider as per the CCF Handbook that ( Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable. E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file. Prior to beginning volunteering in a child care facility, a CF-FSP 5217, Volunteer Acknowledgment which is incorporated by reference in 65C-22.001(7)(e), F.A.C., and may be obtained from the Departments website www.myflfamilies.com/childcare, must be completed and on file at the child care facility for the volunteer.
