The temperature for food delivered from 6/8/2026 to 6/11/2026 was not logged and temperature was not either. Technical assistance was provided that A log must be maintained for all prepared meals being transported into the facility. The log must be retained for a minimum of 12 months. The log must include the delivery date, time of arrival, quantity and types of food, verification by the recipient of adequate temperatures of food, and the name and signature of the recipient. Employment history check did not include all required information including applicants, job title, description of regular duties, confirmation of employment dates, and level of job performance. CCF Handbook, Section 5.1, D (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing specialist added 3 staff files that had employment history verification incomplete by the provider. The staff filled out the information. Provider must complete employment verifications. Technical assistance was provided that The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. The facility did not have documented proof of an annual fire inspection by the local fire authority. CCF Handbook, Section 3.8.2, A Physical Environment [SR] Comments: Licensing specialist reviewed most recent fire inspection and it was on November 4th, 2024. Per provider, she called and they told her they are backed up working July/ August inspections. Technical assistance was provided that To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program. Extended due date provided to accommodate the backed up inspection from the fire department. Last fire drill was conducted on 10/14/2025. One fire drill using an alternate evacuation route has not been conducted during licensure year. One fire drill during napping/sleeping times has not been conducted during licensure year. One fire drill in the presence and at the request of the licensing authority has not been conducted during licensure year. One emergency LOCKDOWN drill has not been conducted during licensure year. One emergency INCLEMENT WEATHER drill was done on 9/24/2025. Fire inspection conducted on 11/4/2024. Fire extinguisher serviced on 04/2025. As technical assistance, the licensing counselor requested that a Fire Drill be done in the presence of licensing authority, and the provider requested that the fire drill be done at the next scheduled inspection. A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [8] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Upon observing ratios at time of arrival in the facility, licensing specialist observed one staff with 8 children in the 1-year old classroom. Ratio at Time of Arrival: Infants - 2:6 1-Year Old - 1:8 2-Years Old - 2:14 (additional staff having lunch in break room on-site) 2-Years Old - 1:9 3-Years Old - 2:14 4-Years and 5-Years Old - 1:10 Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Nap time supervision does not apply to children up to 24 months of age, who must have direct supervision at all times. Licensing specialist observed various staff having lunch in the break room on-site meeting nap time supervision for classes of children 2- years and older. The additional staff from the 1-year old class was in the lunchroom at time of inspection. Upon completion of her break, she returned to the classroom and the standard returned to compliance at time of inspection.
