The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Comments: At the time of the inspection, the licensing specialist reviewed all staff files and determined the staff listed on the supplemental had an expired Child Abuse & Neglect form in their file. The staff signed one while the licensing specialist was present placing the facility back into compliance for this standard. The provider was reminded this form must be signed annually on or before the prior year's date.
