   Supervision of children in the [Two year old ] group was inadequate in that [A child wrapped wrist in rubber bands which teacher neverobserved whil caring for children]. CCF Handbook, Section 2.4 Comments: At the time of the unannounced complaint investigation, it was noted by CCR that the staff failed too adequately to supervise children in groups. After reviewing video of the interaction, the child was able to go under table, place several rubber bands on wrist and the teacher never witnessed the rubber bands on the child which resulted in injuries to the child. The facility was forth coming in allowing access to the video for review to the department to assist in the complaint investigation. At this time the facility will be placed back into compliance as the incident has already occurred and has been reported by the facility to the department.    The facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A Comments: At the time of the inspection, it was noted that the facility did not have a current fire safety inspection by the local fire authority. The facility will be placed back into compliance with the submission of required inspection to the department for review. The facility will be placed on a provisional license at this time until placed back into compliance.
