The facility's transportation log did not include [Arrival and departure times. ]. CCF Handbook, Section 2.5.2 (Section 2.1 Health and Safety, Page 3) General Requirements [SR] Comments: At the time of the inspection, it was noted that that the facilitys transportation log did not include arrival and departure times on multiple days in the transportation log. The facility came back into compliance with correcting the logs with times at the time of the renewal inspection. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the childcare facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the childcare personnel), and 5. The signature of a second childcare personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Counselor asked provider to turn on the vehicle alarm while standing outside of the vehicle. Counselor observed provider walked back of the vehicle to turn the alarm off. The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: At the time of the inspection, it was noted by CCR that the base of the outdoor play area was not at ground level. This was corrected at the time of the inspection with the facility covering the base of the fence with a board and mulch. Please see attached picture. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided. Childcare facility handbook page 24. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 (Section 2.1 Health and Safety, Page 3) Comments: At the time of the inspection, it was noted that one staff did not complete 40-hour introductory training requirement. Please see employee name on confidential page of the inspection. The facility will be placed back into compliance with the submission of required training to the department for review. Childcare personnel must successfully complete 40 hours of childcare training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Childcare personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004, are not required to fulfill the competency examination requirement.
