Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Health Requirements [SR] Comments: On 10/15/2025 Child care personnel failed to document an accident or incident on the day that it occurred when a child was outside on the facility playground and was bitten by ants. When the child came inside neither staff member noticed any ant bites on the child. Only one staff member was aware of the ants on the child and the staff member failed to report the incident to the director and the parents the same day the incident occurred. The director has issued a training for all staff on the procedures of when to fill out an incident report. Documentation of Employee Signatures from training was received by licensing via email. Technical Assistance A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur.
