46. Daily Attendance CCF Handbook, Section 7.5 (46-02)

Attendance records did not include the time of each childs arrival and departure. CCF Handbook, Section 7.5 Comments: On 6/10/26, a staff member failed to accurately conduct a name to face attendance on attendance device used. This resulted in a child being left in the outside playground for 25 seconds. The child knocked on window to get back into classroom. Child unharmed. Staff member was terminated and all staff were retrained in using and accurately recording on device used for attendance and transitions. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the childcare personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the childcare personnel and the group of children throughout the day should they leave the classroom. 04. Supervision CCF Handbook, Section 2.4 (04-10)

A child was left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location away from the facility. CCF Handbook, Section 2.4.1, B Comments: A 2-year-old child left premise of outside play space and staff where unaware child was missing for 5 minutes. Child was able to push outside play space gate open and go into an unfenced area. The space has several hazards that may have endangered child in question. There was a parking lot for another business, a busy road and a water feature only fenced on one side facing the front of facility. The other business, road and water feature do not belong to facility but is rather an easement to facility property. Parents were notified and an incident report was completed and signed. Child is still enrolled and in attendance at facility as of 3.23.26. The Child Care Facility Handbook states on page 12: 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. All Amazing Explorers Academy schools have immediately implemented an additional alarm-testing protocol as part of the daily morning routine. The first administrator on site will now test every alarmed door daily to confirm it is functioning properly. Amazing Explorers Academy corporate team is also in the process of ordering new delayed push bar doors, where the alarm will sound for 15 seconds prior to the latch unlocking. 11. Child Discipline CCF Handbook, Section 2.8 (11-03)

Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B Comments: Facility video reviewed for 1/21/26 and 1/22/26 showed a staff member that inappropriately grab childs arms and got in face of a 14-month -old child after he accidently bit her. Child did not visibly cry however walked away from this staff member. The next day, 1/22/26, this staff member was sitting at the end of the fabric play tunnel, The child tried climbing on the tunnel. She picked him up with two hands on the trunk of his body and sat him down firmly on his bottom next to her. When child then stood up and tried to walk toward the tunnel again, this staff member grabbed his arm and pushed him on the floor. Child then got up again and tried to go towards the tunnel. This time, this staff member quickly put her arm out and pushed child to the floor. Child did not cry and was not hurt physically. The facility has a policy that states age developmentally appropriate redirection will be practiced and implemented which this staff member did not follow. This staff member was terminated form her position at facility 1/26/26.2.8 Child Discipline page 19B. Supervision of children in the [4 to 5 year old] group was inadequate in that [during an end of day transition, 1 child was left in bathroom/classroom without adult supervision. ]. CCF Handbook, Section 2.4 Comments: On 8/26/25, during an end of day transition, a 4-year-old was left in bathroom/classroom for approximately 2 minutes until a parent walked by and saw child in classroom by himself. Parent brought child to his transitioned class. Classroom staff immediately informed administration. Administrative staff viewed video and allegation was valid. Child was observed not to be in distress. Parents were notified and an incident report was generated. Staff member responsible was given a written warning.
