The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. Children in the outdoor play area were not adequately supervised in that [as a child was able to go in and out of classroom twice without out being seen by staff in outside play space. ]. CCF Handbook, Section 2.4.1, A Comments: On 9/8/25, a child was able to slip in and out of classroom twice without direct supervision from staff responsible for him. This was reported by a parent watching live feed of classroom. This was evident as video observed verified child did go in and out of classroom from outside play space. Staff member responsible for child was terminated. A mandatory staff meeting was held to address direct supervision of children. All current staff received a 1-hour in-service certificate for training on 9/24/25. 2.4 Supervision page 12 2.4.1 General Supervision Requirements A. Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Comments: A 2-year-old child left facility with a limp on 4.22.25. It was discovered during investigation that a virus had caused child to limp and have swelling on knee. Child was uncomfortable and not bearing weight at times on her leg between 4.22.24 and 4.24.25. Incidents reports were dated 4.22.25, 4.23.25 and 4.24.25 but were not generated until 4.28.25 and signed by parent on 4.30.25. 6.4 Accident/Incident Notification page 55 and 56 A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Both staff received written warning and retraining.
