CCF Handbook, Section 7.4 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: The licensing specialist reviewed three employee files (for employees TH, HL, and JB) that had missing elements on their 5131 forms. Each employee was missing their social security number, date of birth, and date of hire. The director filled in the information during the inspection, putting this facility back into compliance. The Child Care Facility handbook states: 7.4.1 Background Screening Documents A. Each personnel record must have a completed CFFSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C.
