Indoor and outdoor play areas were not inspected daily for basic health and safety and/or any problems were not corrected before the play area is used by children and/or daily inspection logs were not maintained for 12 months. Comments: The inspection log was not signed for multiple days this week. The SMIC was unaware of the log, which is typically maintained by the Director. The adult-child ratio was not met. There was/were [1] staff supervising [25 children from VPK, K and grade 1. ] children. (Section 3, numbers 1 9) Comments: Due to a staff member being absent, one staff member took 25 VPK, Kindergarten and First Grade outside, which put her over ratio by 5.
