The childrens center did not have a fire extinguisher with a current certificate. (Section 14.2, number 3) Comments: The three vehicles used to transport children did not have fire extinguishers with current certificates. Ensure the fire extinguishers are inspected and tagged appropriately. Submit proof to the License Board office upon receipt. A fire drill was observed during the inspection. Seven adults and 48 children successfully evacuated the building in 1 minute 27 seconds. All children were accounted for and proper procedures were followed. Comments: For morning snack, Toddlers were observed eating quarter apple wedges with the skin and crackers, which posed a choking hazard. Comments: The brochure was distributed to parents in October 2024 instead of September 2024. The adult-child ratio was not met. There was/were [1] staff supervising [6 one-year-old] children. Comments: One staff member was observed alone in the toddler classroom with six one-year old children. The staff member was alone from approximately 9:50 a.m. until 10:00 a.m. until the second teacher arrived. Submit a Corrective Action Plan to the License Board office which ensures adult-child ratios have been reviewed with all staff and how they will be maintained at all times.
