Comments: Nine staff did not complete the Refresher Training before the expiration date of 3-21-25. The Director was able to have the training completed during the inspection. The childrens center failed to report a suspected communicable disease for two or more children or adults within 72 hours to the local community health department and failed to follow directions given. Comments: There have been multiple documented cases of hand/foot/mouth disease at the center, and it was not reported to the health department. The center has taken precautions to notify families of symptoms and obtained doctor's notes for returning students.
