The black metal bars/railing of climber were not maintained in a safe and sanitary condition for children to use due to the black paint chipping/peeling on the play equipment. TA was given to provider to maintain outdoor play equipment free from chipped / peeling paint. Counselor observed an exposed rock near the climber.
