Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Health Requirements [SR] Comments: The facility did not have documentation of the incident that occurred with a staff members child. This was corrected at the time of investigation in that, the new director is aware that all incidents/accidents rather staff or non staff member must be documented the day of the incident/accident. The facility has taken incident/accident writing training and will continue to document incidents/accidents. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. CCF Handbook, Section 6.4, A Health Requirements [SR] Comments: The child's teacher C.M. admitted that she did not write a written incident report when the child C.L. was bitten but she did verbally tell the parent of the child he was bitten. Corrective action: The Director M.F. stated that she would have a facility meeting and discuss with staff the importance of documentation of incident/accident standards in an effort to make sure that all staff are documenting all incident/accident that occur at the facility. Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Enforcement [SR] Comments: Teacher H.P. was observed on video inappropriately with child L.C. when she aggressively grabbed child L.C. underneath his arms and forcibly sitting him in his chair. 8.2 Child Safety A. Acts or omissions that meet the definition of child abuse or neglect provided in Chapter 39, F.S. or Chapter 827, F.S., constitute a violation of the standards is section 402.301-.319, F.S., and will support imposition of a sanction, as provided in Section 402.310, F.S. B. Failure to perform the duties of a mandatory reporter pursuant to Section 39.201, F.S., constitutes a violation of the standards in Section 402.301-.319, F.S. C. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited.
