   A medication, [Nystatin], brought to the center for administration, was not in its original container. CCF Handbook, Section 6.5, C (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: Nystatin was received by teacher from parent with medication form filled out by not in the original container. This was resolved when provider threw the medicine away. Technical Assistance given from the handbook: Medication 6.5 C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. Corrective Action: Provider is to have a staff meeting and retrain all staff about medication and provider will submit an outline of the training along with all staff members signatures.    A child was given [yogurt] after the facility was notified of a food allergy by the custodial parent or legal guardian. CCF Handbook, Section 3.9.3, G Food and Nutrition [SR] Comments: A child at the center was given yogurt for a snack when on the enrollment form and doctor's note provided clearly stated that child is allergic to diary. Technical Assistance given from the handbook: G. If a special diet is required for a child by a physician, a copy of the physicians order, a copy of the diet, and a sample meal plan for the special diet must be maintained in the childs file and followed. If the custodial parent or legal guardian notifies the program of any known food allergies, written documentation must be maintained in the childs file for as long as the child is in care. Special food restrictions must be shared with childcare personnel posted in an easily seen location that is not readily visible by parents or nonchild care personnel and followed. Corrective Action: Provider will need to submit in writing a plan to make sure allergies are known certainly by administrators and by staff in each room including subs and how they plan to update all lists and how often they plan to update allergy lists. Provider will need to have a staff meeting and have all teachers sign and submit via email teachers' signatures and outline of the trainings for allergies.
