Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B General Requirements [SR] Comments: Teacher is the two-year-old room admitted to flicking two children whose parents work there on the head or hand. Parents admitted to giving her permission to do that. Teacher has been terminated. Corrective Action: Provider is to have a staff meeting with all staff and go over the information that is found in Basic Guidance and Discipline and remind staff what their discipline policy is. Provider is to send via email a sign in sheet with all staffs signatures and an outline of what was reviewed. Provider must send this via email by 06/07/26. The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Comments: Provider had an employee listed on the supplemental page that did not start a DCF required class within 90 days of employment; however, before the inspection was over the employee did start a required training. Technical Assistance given from their handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Supervision of children in the [the three year old classroom] group was inadequate in that [the teacher walked the children out of her classroom going outside and left one child behind in the classroom with the door shut]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: On the day this happened the teacher left a child alone in the classroom with the door shut while she walked the class to the double doors that lead out to the playground. The child was heard screaming and crying by other staff members and the child was taken to another teacher in the building who comforted the child and cleaned the child up. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: The child who is three years old was in line to use the bathroom and another child was able to slam his finger into the door because the door locks were broken at the time of the incident according to multiple staff. At the time of the complaint the locks on the bathroom doors were fixed. Technical Assistance given: 3.1 General Health and Safety Requirements A.
