Supervision of children in the [Preschool] group was inadequate in that [the teacher was not in eye sight of children in the loft area of the classroom]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: Licensing Specialist reviewed camera footage of incident. Licensing Specialist observed the teacher in the restroom assisting other children. Licensing Specialist observed two children in the upper loft area unsupervised, and teacher was not aware of what they were doing. Licensing Specialist observed the two children engaged in inappropriate behavior. Technical assistance provided from the CCF Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective Action: Director advised Licensing Specialist that she documented the incident to both parents and she immediately called the parents to advise of the incident. Director advised Licensing Specialist she added additional staff to classroom during restroom breaks. Director also blocked staircase to not allow usage of loft area. Director advised that access to loft area will only be used when two staff members are present. Director provided an Action Plan for proper supervision during restroom breaks to Licensing Specialist. Director provided signed incident reports from both parents. Director separated the students and relocated them to separate classrooms. The standard is brought into compliance. CCF Handbook, Section 2.4 General Requirements [SR] Comments: Licensing Specialist received a report of children biting in the one-year-old classroom. Licensing Specialist interviewed staff and determined a lack of supervision during diaper changes and playground time. Technical assistance provided from the CCP Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Corrective action: Provider had meeting with the one-year-old classroom to discuss recent issue with biting and new practices to illuminate future incidents. A method of discipline was used at the facility that was severe, humiliating or frightening to children in that [a staff member used extreme force to make a child stay on their assigned sleeping mat during naptime.]. CCF Handbook, Section 2.8, A and s.402.305 (12), F.S. General Requirements [SR] Comments: Licensing Specialist reviewed camera footage provided by Director. Licensing Specialist observed staff member lift the child with one arm from the ground and body slam the child on the sleeping mat several times during the observation. Licensing Specialist observed the staff member continuously applying pressure to the childs head with her hand with force and with harsh verbal tones. Licensing Specialist observed staff members completely covering the childs head with a blanket during naptime. Licensing Specialist observed the staff member placing her leg over the childs back to restrain the child from moving and getting off the sleeping mat Technical assistance provided from Florida Statutes: s402.305 (12), FS - Licensing standards; child care facilities. (12) CHILD DISCIPLINE. (a) Minimum standards for child discipline practices shall ensure that age-appropriate, constructive disciplinary practices are used for children in care. Such standards shall include at least the following requirements: 1. Children shall not be subjected to discipline which is severe, humiliating, or frightening. 2. Discipline shall not be associated with food, rest, or toileting. 3. Spanking or any other form of physical punishment is prohibited. (b) Prior to admission of a child to a child care facility, the facility shall notify the parents in writing of the disciplinary practices used by the facility. Corrective Action: Director advised that she have a staff meeting scheduled to review and discuss appropriate discipline practices, child abuse reporting, supervision, cell phone usage and safe sleep practices. Provider is to submit a corrective action plan outlining discipline and supervision policies to preventing this from happening again. Hands must be washed and dried thoroughly after each diapering or toileting procedure, and all surfaces that have been touched must be cleaned and sanitized or disinfected to prevent the spread of germs. C. Handwashing sinks must not be used for food service preparation, dishwashing, or food clean up. D. The diaper changing area must be physically separated from the food preparation, food service, and feeding area. E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. F. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given.
