An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Comments: Licensing Specialist observed paint chipping in 2 bathrooms which are utilized by children ages 3-years-old through school age. Licensing Specialist observed paint chipping more than three and half inches in length around sink in bathroom. Licensing Specialist observed paint chipping more than three and half inches in length above toilet lids in 2 bathrooms. Areas observed with chipping paint are within the reach of the children who utilize the 2 bathrooms. Photos taken of both bathroom areas where paint is chipping. Corrective action needed. CCF Handbook, Section 7.1 Record Keeping [SR] Comments: Licensing Specialist reviewed 14 childrens files (L-Z) including newly enrolled. Child listed on supplemental did not have a current immunization record on file. See supplemental. Corrective action needed. Technical assistance was given regarding maintaining current immunization documentation for all children enrolled at the facility. Per CCF Handbook, Section 7.1The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Per CCF Handbook, Section 7.2 The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination (page 60). The facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [the facility did not have documentation readily accessible for Licensing Specialist to monitor for the month of April 2025.]. CCF Handbook, Section 7.3, C.5. Record Keeping [SR] Comments: The facility did not have documentation readily accessible for Licensing Specialist to monitor for the month of April 2025. Corrective action needed. Technical assistance was provided regarding maintaining documentation that all parents/legal guardians at the facility received the Distracted Adult Brochure for the months of April and September annually. Provider stated she could not locate the documentation at the time of the inspection. Provider stated documentation was sent out on 04/07/25.
