Supervision of children in the [three-year-old] group was inadequate in that [the teacher was distracted by a laptop while being the only staff member in the classroom supervising the children]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: Video was reviewed I observed a video that was emailed to me from the facility, The teacher was the only staff member in the classroom at the time of the incident; she was distracted by a laptop and should have given the children more attention. The children could not be observed in the area behind of the shelf in the classroom, teacher should be making periodic checks to ensure the safety of the children. Corrected: I spoke with the facility director and advised her to communicate to the facility staff about distractions and children being in line sight especially when they are the only staff member present. 2.4.1 General Supervision Requirements A. Childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. 05. The ground cover or other protective surface under the [Climber and slides] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: Specialist measured the fall zones at the time of inspection and found it to be inch in depth. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. A safety deficiency having a low potential for harm to the children in care was observed. CCF Handbook, Section 3.1 (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: There was a baby in care crawling around the infant room with a bib dragging on the floor this posed a strangulation hazard. This was corrected at the time of inspection teacher removed the bid from the child immediately. 3.1 General Health and Safety Requirements D. Strangulation hazards, strings, and cords long enough to encircle a childs neck such as those on toys and window coverings must not be accessible to children in care. 13.
